Settling in Dubai — questions

I'm processing a visa for a housekeeper and I don't know where the medical test has to be done. I've heard the cost varies a lot between clinics so I want to look into it in advance, and I'm also curious whether public or private is the better option.

3 months since I landed in Dubai. Got two quotes for house cleaning and the price gap threw me. One says base rate plus an add-on for bathrooms, the other says bathrooms are included, and I don't know which is right. Are cleaning quotes here normally this different? If anyone knows, please fill me in.
